Well if you are talking about his last meal in Hobbiton, in Bag End, then it was just Frodo and Pippin with Sam, it was tea time, and Lobelia was there with Lotho but they were not offered any, do they count? But if 'familiar lands' are just the Shire in general then his last meal was in Crickhollow with Merry, Pippin, Frodo, and Fatty.

They were in the dell below Weathertop directly after spotting the Black Riders on the road from the top of Weathertop, and Aragorn said:
