Is his dragon fire injury shown in Desolation of Smaug just a memory? or is that his real current face covered by illusion magic?

I'd say it's only a memory. He's an Elf, when that fire was not enough to kill him, he should heal completely from it.

But as he's an Elf, that memory is probably more strong and vivid than for the other peoples of Middle-earth. Visiting a memory for him could be very close to re-living it.

Sounds reasonable, and the Elven memory bit is spot on, yet Maidhros, a more powerful elf, could not regenerate a severed hand. I suspect that Elven healing can be impaired by a marred Fea, which Maidhros suffered a bit from, and which movie Thranduil appears to exhibit to some extent.

That could be a great part of Thranduil's problem.
Celebrian suffered from that as well - it's possible that her body got completely healed, but the marring of her fea couldn't be healed in Middle-earth, so she left for the West.
